What causes this error in stages?

The images for this stage break, or tear, or something,  I dunno what to call it, so here's a photo.  What causes this and how can I fix it?

That normally occurs when you have some of the "default mugen BG" visible. It's black, but when things move across it they afterimage like in that shot. Tweak camera follow to make sure you can't pass beyond the boundaries of the picture and make sure the picture is AT LEAST the same size as the screen.
